I would like to have my game servers (BBSlink, Doorparty) load and run right from the main "Online Program Section" instead of having to go to the next menu to make the selection, is that possible?

yeah it's possible. you just change the code.
you can put a link for them in your main menu too. you can do
almost anything.

I seem to remember that if there is only one item within a menu then
xtrn will run that program automatically so try creating a new group
for each door.

Yes. xtrn_sec.js is just a script you can modify, so in theory, you can do whatever is possible via JS in that script.

However, just via configuration changes, I think you can achieve what you're asking:
- Put each game server in its *own* section in SCFG->External Programs->Online Programs.
- set autoexec=true in the [xtrn_sec] section of your ctrl/modopts.ini file
